COMPENSATION: The hiring range for this position is $70,000-$90,000, with the opportunity to earn additional compensation through bonuses; candidates may also choose between a base salary structure or a variable compensation plan (commission), which may provide higher earning potential

Position Summary
Vitae Health is seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents' overall well-being and adjustment to medical and life changes.
Key Responsibilities
  • Conduct comprehensive psychosocial assessments for residents upon admission and as needed
  • Develop, implement, and monitor individualized treatment plans
  • Provide individual, group, and/or family therapy sessions
  • Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disability
  • Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns
  • Collaborate with the Vitae clinical team, the patient's attending physician, facility nursing staff, social workers, and rehabilitation teams to coordinate care
  • Participate in care plan meetings and interdisciplinary team discussions
  • Maintain accurate and timely clinical documentation in compliance with facility, state, and federal regulations
  • Provide crisis intervention and behavioral management strategies when needed
  • Educate staff and families on mental health conditions and effective communication strategies
  • Ensure compliance with HIPAA and ethical standards of practice

Requirements
  • Master's degree in Psychology, Counseling, Social Work, or related field
  • Active and unrestricted license in respective state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist)
  • 2 years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings
  • Strong clinical assessment, diagnostic, and treatment planning skills
  • Knowledge of mental health conditions common in SNF populations (e.g., depression, anxiety, dementia, adjustment disorders)
  • Proficiency in evidence-based therapeutic approaches (e.g., CBT, trauma-informed care, grief counseling)
  • Ability to work collaboratively within an interdisciplinary team
  • Excellent communication and interpersonal skills with residents, families, and staff
  • Strong organizational, time management, and documentation skills
  • Familiarity with electronic health records (EHR) systems preferred
