• Conduct in-depth clinical assessments to evaluate patient needs, strengths, and challenges, leading to the formulation of personalized treatment plans.
• Implement evidence-based therapeutic interventions focused on solution-oriented practices, addressing both mental and emotional health concerns of patients.
• Maintain accurate and timely documentation of all clinical interactions, adhering to regulatory standards to support quality patient care.
• Collaborate effectively with a multidisciplinary team, including nurses, physicians, and other healthcare providers, to ensure coordinated and comprehensive care for patients.

This position is centered around supporting patients and families facing terminal illnesses. You will work with a diverse range of individuals, each with unique circumstances, including chronic illness cases and complex emotional and psychological needs. Your role will be pivotal in guiding families through grief, understanding the processes involved in end-of-life care, and helping them celebrate and honor their loved ones’ lives.

• A Master's degree in Social Work from an accredited institution is required.
• Possession of a valid Licensed Clinical Social Worker (LCSW) license in the state of Kansas is essential.
