Job description

Overview

The Discharge Planner provides services to patients, families, and others in all hospital areas specific to assignment as applicable, and in accordance with hospital customer service expectations.  Professional services to be provided to meet the discharge planning needs of the patient may include, but are not limited to, case consultation, patient education, resource management, and assistance with discharge planning.

The Discharge Planner is an integral part of the interdisciplinary team, and also provides community outreach, networking, and liaison with community programs as needed.

Responsibilities

Consistently applies infection control policies/practices.

Meets population specific competencies.

Attends department specific education/training, inservices, staff meetings.

Department specific performance improvement project.

Demonstrates ability to provide timely and appropriate discharge planning assessments and interventions for patients .

Demonstrates ability to develop and implement appropriate discharge planning plan of care.

Demonstrates ability to advocate for patient's rights.

Knowledgeable regarding community resources and demonstrates ability to mobilize them on patient's behalf.

Demonstrates ability to maintain appropriate documentation and statistical records.

Qualifications

Bachelor's degree from an accredited school of Social Work or related field (preferred), RN or LVN (preferred)

Two years acute or related medical care experience.
