Job description

Liquid Personnel is recruiting an Adult Social Worker to join its client's Hospital Discharge team, based in Lancashire.

What will your responsibilities be? 

  • Provide statutory social work support - delivering professional social work interventions and fulfilling legal duties under adult social care legislation.
  • Promote independence and recovery - completing strengthsbased assessments and enabling individuals to achieve the best possible outcomes.
  • Coordinate safe hospital discharges - working closely with health partners to ensure timely, appropriate discharge planning and community support.

Qualifications and Experience: 

To be successful in this role you must have;  

  • Social Work England registration 
  • Eligible to work in the UK 
  • Hold a full UK driving licence 
  • Post-qualified experience 
  • Degree level or equivalent in Social Work
