Job description

Utah Case Management is looking to hire a Registered Nurse to join our case management team. The Dual Nurse Case Manager coordinates medical services and coordinates care for our clients through Utah’s Medicaid New Choices Waiver program. We offer the benefit of a flexible work schedule allowing you to govern your own hours and schedule. This is a hybrid position with remote documentation and in person client visits. We are looking for dedicated employees to coordinate services for our clients. This is a dual position encompassing both Nurse and Social Work duties that is located in the St. George, Utah area.

Nurse Responsibilities:

  • Complete Nursing assessments (MDSHC, LOC, and Medication Management Review) within the allotted time frames for each client.
  • Work on a team with a social worker in all healthcare decisions or needs of the client.
  • Perform the health status screening report when clients return from an inpatient setting or have a decline in health status.
  • For clients that need memory care, complete memory care checklist and needed documentation for approval of clients’ to reside in memory care.
  • Attend required nursing training meetings and other trainings as required.

Social Work Responsibilities:

  • Weekly contact with clients, service providers, family of clients, etc.
  • Monthly face to face contact.
  • Assess, coordinate, and manage needed services for clients.
  • All initial care plans in pending or approved areas on the State Online Care Plan system by the 5th of the month following enrollment.
  • All denied care plans need to be reviewed and re-submitted within 48 hours of denial.
  • Complete all required documentation, including audits and wrap up lists.
  • Conduct New Referrals, Annual Reviews, Settings Reviews, etc.

Dual Responsibilities

  • Must be able to accept constructive criticism and direction.
  • Must be able to work independently as well as function as a team player.
  • Must be able to use own computer and phone for work purposes.
  • Have log notes completed by the 15th for the first half of a month and last day of the month for the 2nd half of the month.
  • Attend monthly team meetings and case reviews with supervisors as needed.
  • Respond to the facility on a new referral 24-48 hours upon getting the referral to let them know you are aware of the client and when you will be coming out to meet the client.

Qualifications

  • Utah Resident and residing in St. George, Utah.
  • Current Registered Nurse license in the State of Utah.
  • Good at creating and maintaining rapport with clients.
  • Detail oriented.
  • Self motivated.
  • Good communication and organizational skills.
  • Computer literacy skills.
