JOB SUMMARY

Provides social work case management services to adults in home- and community-based settings according to County, state, and federal regulations, policies, and procedures. Provides information, awareness, marketing, and educational programs for caregivers and the community.

MINIMUM QUALIFICATIONS

Bachelor’s degree from an accredited college or university in Social Work, Gerontology, or other closely related field; OR an equivalent combination of related education and experience. 

Must be licensed as a Social Service Worker in the State of Utah or certified through the National Academy of Certified Care Managers (CMC).

Due to the nature of this position, the successful applicant must pass a required background check through fingerprinting and enrollment in the continuous RAP Back (Record of Arrest and Prosecution Back) program in accordance with current County Human Resources policy requirements.

A valid driver’s license accepted by the State of Utah is required at the time of hire. A valid driver’s license issued by the State of Utah must be obtained within six (6) months of hire to operate a county vehicle.

ESSENTIAL FUNCTIONS

  • Uses acceptable case management practices to complete intakes and comprehensive assessments/reassessments to screen and determine eligibility, detailing presenting client care, training, and safety needs.
  • Develops, implements, monitors, and evaluates comprehensive care plans that address identified client service, training, and safety needs.
  • Creates and maintains accurate clinical and financial records for assigned clients.
  • Manages fiscal expenditures of assigned case load, adjusting clients to maintain desired spending levels.
  • Works closely with contracted service providers to ensure the quality of care and services provided.
  • Provides advocacy on behalf of clients as appropriate.
  • Provides information services and training to groups of individuals in the community related to available resources as needed.

  • Medicaid and Medicare rules and regulations
  • Assessment methodologies and techniques
  • Acceptable case management procedures
  • Crisis intervention strategies and techniques
  • Aging process, chronic illness, and related conditions
  • Community resources, agencies, and health care delivery system rules and regulations
  • Interviewing methods and techniques
  • Program evaluation techniques

IMPORTANT INFORMATION REGARDING THIS POSITION

Part-time position - The Aging Waiver Program is seeking a part-time Case Manager to support individuals who wish to remain living independently in their homes and communities. The Case Manager will provide community-based case management, coordinate in-home services, and connect participants with resources that promote health, safety, and independence.


WORKING CONDITIONS AND PHYSICAL REQUIREMENTS

Case Managers are often subjected to unsanitary conditions, pets, and unsafe neighborhoods.

Must be able to access transportation to perform work assignments in a timely manner.
