Job description

Utah Case Management is looking to hire a Registered Nurse to join our case management team. Nurse Case Manager coordinates medical services for our clients through Utah’s Medicaid New Choices Waiver program. We offer the benefit of a flexible work schedule allowing you to govern your own hours and schedule. This is a hybrid position with remote documentation and in person client visits. We are looking for dedicated employees to coordinate services for our clients. This is a PRN position that is located in SALT LAKE COUNTY

Responsibilities:

  • The main responsibility would involve completing medication management reviews for clients: Obtaining a list of the client's meds, assessing medications, and complete medication management review paperwork.
  • Assessing clients' health condition and identifying supports needed for their health and safety.
  • Managing clients' services: evaluating the services the clients are receiving to make sure they fit the client's health needs, coordinating services for clients, etc.
  • Reviewing clients' care, services, and medication regularly and when a client has had a change in health status.
  • Documentation: completing, updating, submitting, and compiling necessary documentation for clients.
  • Communicating with the social work case manager, facility staff, and service providers for coordinating clients' care.
  • Regularly attend staff meetings and company trainings
  • Able to use your own devices
  • Able to travel to visit clients each month

Qualifications

  • Utah Resident and residing in SALT LAKE COUNTY
  • Current Registered Nurse license in good standing in the State of Utah
  • Good at maintaining relationships with clients
  • Detail oriented
  • Self Motivated
  • Able to work independently and with a team
  • Good communication and organizational skills
