Position Summary:

The Family Tree Program Case Manager is responsible for providing comprehensive case management services to children, adolescents, and families participating in the Family Tree Program. The Case Manager coordinates behavioral health, medical, educational, social, and community-based services to ensure families receive integrated, person-centered, and culturally responsive care.

Working within SAMHSA's principles of recovery, resilience, trauma-informed care, family engagement, and health equity, the Case Manager assists participants in overcoming barriers to treatment, strengthening protective factors, improving family functioning, and increasing access to community resources that promote long-term wellness.

Essential Job Functions:

• Conduct comprehensive strengths-based assessments of participant and family needs.
• Develop individualized service plans in collaboration with participants, families, and the multidisciplinary treatment team.
• Coordinate behavioral health, substance use, primary care, educational, vocational, housing, transportation, and social service supports.
• Monitor clients progress toward established goals and revise service plans as needed.
• Ensure continuity of care across providers and systems.
• Embrace and embody WestCare's mission, vision, guiding principles, clinical vision, and goals.
• Build collaborative relationships with children, youth, caregivers, and family members.

• Psychoeducation and prevention in substance use and risk behaviors. Providing group and individual interventions.
• Encourage active family participation in treatment planning and service delivery.
• Educate families regarding available community resources and support services.
• Promote family resilience, empowerment, and self-advocacy.
• Identify participants experiencing behavioral health crises.
• Coordinate crisis intervention services and safety planning.
• Facilitate referrals to emergency behavioral health services when appropriate.
• Follow up after crisis episodes to ensure continuity of care.
• Maintain timely, accurate, and complete case records.
• Document all clients contacts in accordance with organizational policies and grant requirements.
• Complete assessments, service plans, progress notes, referrals, and discharge documentation within required timeframes.
• Maintain confidentiality consistent with HIPAA and organizational policies.
• Participate in multidisciplinary treatment team meetings.
• Collaborate with therapists, psychiatrists, peer support specialists, schools, healthcare providers, child welfare agencies, juvenile justice, and community partners.
• Advocate for participant needs across systems of care.
• Conduct outreach to participants who miss appointments.
• Utilize engagement strategies to reduce barriers to treatment.
• Promote retention and successful completion of services.
• Assist with collection of program performance measures.
• Participate in quality improvement activities.
• Support SAMHSA reporting requirements and outcome measurement activities.
• Participate in staff meetings and required trainings.
• Complete continuing education related to trauma-informed care, family engagement, behavioral health, and evidence-based practices.
• Perform other duties as assigned.

Education:

Bachelor's degree from an accredited college or university in social work, psychology, counseling, and/or similar course of study, plus six (6) months of professional experience in chemical addiction or mental health counseling. Individuals must be a team player, pay attention to detail, and have a collaborative nature and be willing to travel.

Experience and Competencies:

• Experience working with children, adolescents, and families preferred.
