Location: Collingdale, PA

About the Role

A Nursing Home in Collingdale, PA, is seeking a compassionate and dedicated Social Services Professional to join our team. In this role, you will play an essential part in supporting the emotional, social, and psychological well-being of our residents and their families. You will work collaboratively with residents, families, and the interdisciplinary healthcare team to ensure each resident receives comprehensive, individualized support.

Responsibilities
  • Provide emotional support, counseling, and advocacy to residents and their families.

  • Address residents' social, emotional, and psychological needs.

  • Develop and implement individualized care plans that promote resident well-being, independence, and quality of life.

  • Facilitate communication between residents, families, healthcare providers, and other stakeholders.

  • Conduct psychosocial assessments and evaluations to identify resident needs and develop appropriate interventions.

  • Collaborate with nursing, medical, rehabilitation, and other healthcare professionals to coordinate comprehensive resident care.

  • Assist residents and families with accessing appropriate community resources and support services.

  • Support residents and families through transitions of care, including admissions, discharges, and changes in level of care.

  • Document resident progress, interventions, and services provided while maintaining accurate and confidential records.

  • Maintain compliance with HIPAA regulations and facility policies.

  • Advocate for residents' rights, preferences, and overall quality of life.

Requirements
  • Bachelor's degree in Social Work, Psychology, Human Services, or a related field, or equivalent relevant experience.

  • Minimum of 4 years of experience in social services or a related healthcare field.

  • Experience working with older adults, patients, residents, or families in a healthcare or long-term care setting preferred.

  • Strong communication, interpersonal, organizational, and problem-solving skills.

  • Ability to maintain confidentiality and comply with HIPAA requirements.

  • Compassionate, empathetic, and patient-centered approach.

  • Ability to work effectively both independently and as part of an interdisciplinary team.

  • Strong documentation and follow-up skills.
