Outfield Healthcare Partners is seeking a dedicated and compassionate Social Services Director (LSW) to join our leadership team in Plainfield, Indiana. What You’ll Do

As the Social Services Director, you will oversee the facility’s Social Services program and ensure residents receive the emotional, psychosocial, and advocacy support they need to thrive.

Primary Responsibilities

  • Lead and manage the Social Services department in compliance with federal, state, and company guidelines

  • Complete social histories, psychosocial assessments, care plans, and required documentation

  • Coordinate discharge planning and community resource referrals

  • Support residents and families with transition planning, emotional support, and resident rights advocacy

  • Participate in interdisciplinary care plan meetings and quality assurance initiatives

  • Assist residents with Advance Directives and psychosocial needs

  • Collaborate with hospitals, hospice agencies, outpatient providers, and community organizations

  • Maintain accurate documentation within PointClickCare and other systems

  • Educate residents, families, and staff on resident rights, abuse prevention, and psychosocial care

  • Prepare the department for state survey readiness and compliance

Qualifications

Required

  • Licensed Social Worker (LSW) in the State of Indiana

  • Bachelor’s degree in social work or human services-related field

  • Minimum 1 year of supervisory social work experience in a healthcare setting

  • Experience working with geriatric residents

  • Strong understanding of discharge planning, care plans, and psychosocial assessments

Preferred

  • Skilled Nursing Facility (SNF) or Long-Term Care experience

  • Experience with PointClickCare or electronic medical records

  • Knowledge of Medicare, Medicaid, PASRR, and state/federal regulations
