Service Coordinator
My Health My Resources in Tarrant County (MHMR) is looking for a skilled Service Coordinator. In Tarrant County, individuals receiving services under HCS, TxHmL, and GR CFC programs are each assigned a Service Coordinator (SC). The SC assists individuals in identifying and expressing their goals, needs, and preferences, ensuring that services align with their Person Directed Plan (PDP). Disability Services aims to deliver consumer-driven services seamlessly, and the SC plays a crucial role in coordinating and monitoring these services. Additionally, the SC must possess knowledge of available community services and supports.
Minimum qualifications: Bachelor's degree in applied Behavior Analysis, Behavioral Services field, Communication Disorders, Criminal Justice, Counseling, Education, Family Studies, Gerontology, Educational Psychology, Health Services, Human Services field, Human Growth and Development, Medicine, Nursing, Physician Assistant, Psychology, Rehabilitation, Social Services field, Social Work, Sociology, and Special Education
Preferences: Spanish fluency
License/Certifications: Valid Texas Driver's License
