The Banyan at Montclair is seeking a compassionate, organized, and resident-focused Social Services Assistant to join our interdisciplinary team. This position plays an important role in supporting the emotional, psychosocial, and discharge planning needs of our residents while helping create a welcoming and supportive environment for residents and their families.

Position Summary

The Social Services Assistant works under the direction of the Social Services Director to assist in meeting the medically related emotional, social, and psychosocial needs of residents. This position supports admissions, care planning, discharge planning, documentation, resident and family communication, and compliance with federal, state, and local regulations. This posting is based on the attached job description with The Banyan at Montclair branding replacing Azria Health.

Essential Duties & Responsibilities
  • Assist with resident admissions, transfers, and discharge planning.
  • Welcome new residents and families and provide orientation to facility services and resident rights.
  • Maintain accurate resident social service documentation in accordance with federal, state, and facility requirements.
  • Participate in interdisciplinary care plan meetings and resident care conferences.
  • Assist residents and families in identifying community resources and support services.
  • Help coordinate resident belongings inventory during admission.
  • Provide emotional support and informal counseling to residents as appropriate.
  • Work collaboratively with nursing, therapy, activities, dietary, and other departments.
  • Help resolve resident and family concerns professionally and compassionately.
  • Maintain confidentiality of resident information in accordance with HIPAA regulations.
  • Complete assigned documentation accurately and within required timeframes.
  • Attend required meetings, education programs, and in-services.
  • Perform other duties as assigned by the Social Services Director or Administrator.
Qualifications
  • Associate degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred.
  • Relevant long-term care or healthcare experience may be considered in place of formal education.
  • Knowledge of state and federal long-term care regulations preferred.
  • Excellent communication, organizational, and documentation skills.
  • Strong computer skills, including Microsoft Office.
  • Ability to work effectively with residents, families, physicians, and interdisciplinary team members.
Physical Requirements
  • Frequent walking throughout the facility.
  • Ability to stand, bend, reach, and lift up to 15 pounds.
  • Ability to remain calm and professional in stressful situations.
  • Ability to communicate effectively both verbally and in writing.
