Job description

Responsibilities

The Social Worker Assistant (SWA) supports the administrative activities of the Social Work staff in accordance with InnovAge policies and all applicable regulations.  The SWA will work under the supervision  and direction of the Social Services Supervisor.  The tasks assigned to the SWA may not include activities required by regulation or contract requiring or describing a Social Worker and requiring a Masters of Social Work or other requirements such as care planning, case management, assessments, psycho-social services.   The tasks assigned to the SWA may include but are not limited to non-skilled, routine administrative activities necessary to ensure that participant needs are being addressed such as  confirming the delivery of goods and services or start date of external services and supports ordered by the IDT as part of the Care Plan; assisting participants with Medicaid recertification; researching/identifying community-based agencies to address social determinants of health or other participant needs; other tasks as directed by the Social Services Supervisor.

Essential Functions and Work Responsibilities

Functional Category: Social Work Support Activities - 100%

Goods, Services and Supports Fulfillment:  The SWA may be assigned tasks that include contacting participants to confirm the delivery of critical DME, clinically indicated goods, services, or supplies and other interventions critical to supporting the participant in his/her setting. 

Benefit Program Application Support:  The SWA may be assigned to  provide hands on assistance to participants with applying for or recertifying for Medicaid/Medical, Medicare  in collaboration with the Eligibility and Finance Department.  Additionally, the Social Work Assistant may provide hands on assistance to participants applying for housing, homelessness prevention, food security or other similar programs identified by participants' InnovAge Social Worker as a needed intervention.  Administrative support for end-of-life coordination may be assigned as directed by the Social Work Supervisor.  Assists with housing encounters documentation in EPIC

Community Resource Research:  The SWA may be assigned to research, identify and catalogue community or faith-based organizations; city, county or state programs; other services to address the needs of individual participants or participant populations within the Center.  This would include services provided, contact person, hours of operation, and contact information necessary for the assigned Social Worker to make referrals and/or for participants to self-refer.

Outreach: The SWA may be assigned tasks related to locating and validating participant location, telephone number, address, emergency contacts and other demographic information necessary for ongoing engagement with the participant.  Additionally, at the direction of the Social Work Supervisor, the SWA may be responsible for updating key demographic information in InnovAge systems as defined by policy or regulatory requirements.

Qualifications

REQUIRED

Education 

  • Community Health Worker training or certification course, Associate's degree in healthcare, social services, or other equivalent training;  Bachelor's of Social Work, Health Care Administration, or other equivalent training preferred.

Licensure, Certification, Registration or Designation 

  • Require personal transportation, current state issued driver's license, good driving record and auto insurance as required by law.
  • CPR/BLS and First Aid required in California only.

Work Experience and Qualifications 

  • Two years' experience in community-based social services organization or health care setting required.
  • Experience in connecting clients with home and community services, applying for/recertifying Medicaid/Medical eligibility, performing client fulfillment activities, or other similar experience required.
  • Requires personal transportation, current state issued driver's license, good driving record and auto insurance as required by law.
  • A minimum of one year experience working with older adults and people with disabilities 

PREFERRED

Work Experience and Qualifications 

  • Three years or more of experience working in human, healthcare, or community-based services for older adults and people with disabilities.
