Job description

About this Job Case Manager for skilled and intermediate care for Medicare veterans. Assist veterans and their family members through admissions and financial affairs. Assisting the Social Service staff.

Our mission at the Louisiana Veterans Homes is to serve the Veteran population of Louisiana through the provision of social services in an atmosphere of respect and compassion. Join the State of Louisiana Department of Veterans Affairs at the Southeast Louisiana Veterans Home, where you'll step into a role as a SOCIAL SERVICE COUNSELOR 3 employee that combines social service and excellence with genuine care for those who served. An ideal candidate must possess the following: Accepting Direction: The ability to be open and willing to follow guidance or instructions.

Communicating Effectively: The ability to convey information, ideas, and emotions using structured communication methods that promote understanding and engagement. Acting Decisively: The ability to make timely, confident decisions using sound reason. Acting with Ethics and Integrity: The ability to make choices that reflect ethical standards, integrity, and honesty, regardless of circumstances or personal benefit.

Adapting to Change: The ability to demonstrate flexibility in thoughts, behaviors, and actions in response to evolving circumstances or unexpected change. Building and Supporting Teams: The ability to combine one's actions and efforts with those of others to work toward achieving a common goal. Building Trust: The ability to create a safe and supportive environment where others feel comfortable speaking up, sharing ideas, and acknowledging mistakes.

Minimum Qualifications Five years of social services experience; OR Six years of full-time experience in any field plus two years of social services experience; OR A bachelor's degree plus two years of social services experience; OR An advanced degree in a social sciences field plus one year of social services experience. EXPERIENCE SUBSTITUTION: Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

NECESSARY SPECIAL REQUIREMENT: Positions that provide addictive disorder counseling or prevention services will require certification or eligibility for certification issued by the Addictive Disorder Regulatory Authority (ADRA), or its successor. Job Specification The official job specifications for this role, as defined by the State Civil Service, can be found here. Job Duties and Other Information Job Duties: Serves as Case Manager for medically complex, intermediate, and skilled care, including Medicare veteran units with a caseload of veterans with multiple psychiatric, substance abuse, behavioral, and medical problems.

Resident care in a 156-bed long-term care facility. May escort and assist veterans to medical and other appointments. Responsible for admissions protocol involving financial affairs, medical records, and placement on their assigned units, and assist other Social Service staff.

Investigates and reports resident complaints and incidents, recommending corrective actions and collaborating with the supervisor. Updating the veterans' face sheet represents the department at monthly Residents meetings.
