Job description

JOB DESCRIPTION - Social Worker (QIDP)-Qualified Intellectual Disabilities Professional


REPORTS TO: Program Director

FUNCTION: The focus of the QIDP practice is the interactions between people and systems in a residential setting.

JOB REQUIREMENTS AND RESPONSIBILITIES: This position requires at least a Bachelor's degree in a human service related field, also, where appropriate, designation of QIDP status by Federal and State guidelines. This position requires the ability to:

monitor data collection

collect data and evaluate data to determine progress

train direct care staff members

observe residents behavior

exercise good logical judgement in the area of crisis intervention

This position requires a broad range of knowledge regarding philosophy and principles of normalization and policies and procedures as they relate to handicapped individuals, e.g., juvenile delinquents, foster care, mental retardation and mental illness.

PRINCIPLE ACTIVITIES:

Participates in the pre-placement interview to assist in determining the appropriateness of potential residents.

Coordinates the intake of new admissions in order to ensure that arrangements for the needed program services and financial resources are made.

Monitors the development, periodic review, implementation and updating of the Individual Program Plan (IPP).

Implements the Individual Program Plan (IPP) and its related documents.

Assists direct care staff in understanding the needs of the residents and family through ongoing contact and in-service training.

Assist home manager in planning, organizing and coordinating resident's meetings in order to ensure that sessions promote greater independent for residents.

Advises direct care staff in record keeping requirement, practices, and procedures.

Monitors the implementation of the Individual Program Plans, and ensures that they are used to maximize each resident's progress through the use of a Monthly Review.

Follow-up on professional evaluations, reports to Program Director and Interdisciplinary Team members the results of ongoing programming and services to enable the team to develop and maintain a comprehensive individual plan for the resident.

Attend training and other developmental sessions to continue to improve job performance.

Conducts investigations where there are allegations of abuse, mistreatment, and neglect and prepare reports to Executive Director, Assistant Executive Director, Program Director, and external monitoring agencies on all such investigations.

Other duties as assigned.

This is a full time position.
