Social Worker – Patient and Family Support (Asicc-Med – Mobile Health) Willing to work in the area mentioned and ideally owns a means of transport. Ability to work in one of the following settings: CHSLD, CLSC, hospital (CH) or private setting. Location: Quebec, Canada (home and partner establishments) Contract type: Full-time or part-time (permanent/fixed-term contract depending on needs) Schedule: Day, evening or night depending on assignments and availability Compensation: Up to $46.50 per assignment + bonuses, transportation reimbursement, continuing education About Asicc-Med – Mobile Health: At Asicc-Med, we believe that health is not limited to medical care: it also includes psychological, social, and family well-being. Our social workers support patients and their loved ones with the challenges related to loss of independence, care transitions, and access to community resources. Your responsibilities: Assess the social, family and financial situation of the patients. To support individuals and their families in accessing community and public resources. Supporting families in decisions related to home care, accommodation, or specialized care. To offer active listening and appropriate psychosocial support. Participate in interdisciplinary care planning with the clinical team (nurses, PABs, doctors, etc.). Intervening in crisis situations to support patients and families. Maintain up-to-date social records in accordance with professional standards. Desired profile: Bachelor's degree in social work or related discipline recognized in Quebec. Member of the OTSTCFQ (Order of Social Workers and Marriage and Family Therapists of Quebec). Experience in healthcare, gerontology or home care (an asset). Excellent communication, listening and helping relationship skills. Ability to work independently and in an interdisciplinary team. Sense of ethics, confidentiality and organization.
