We are currently recruiting for a Social Services Member to join our team!

Duties and Responsibilities: Social Services

  • Assist in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of the facility.
  • Attend meetings with administration, medical, and nursing staff as directed.
  • Assisting and reviewing the development, administering, and coordinating of department policies and procedures.
  • Participate in community planning related to the interests of the facility and the services and needs of the resident and families.
  • Participate in discharge planning, development, and implementation of social care plans and resident assessments.
  • Refer resident/family to appropriate social service agencies when the facility does not provide the services or needs of the resident.
  • Maintain a quality working relationship with the medical profession and other health-related facilities and organizations.

Qualifications and Skills: Social Services Director

We are seeking team members who have the following qualifications and skills:

  • Professional enthusiastic attitude
  • Bachelor’s or Master’s degree in Social Work, Psychology, or a related field.
  • Licensed Social Worker (LSW) or equivalent certification preferred.
  • Experience in long-term care or geriatric social services is highly desirable.
  • Strong communication skills
  • Self-motivated
  • Shows initiative in daily work
