Bienvivir All-Inclusive Senior Health ("Bienvivir") is a community-based, patient-centered, comprehensive health care delivery system that advocates and promotes quality of life, optimum independence, dignity, and choices in a nurturing environment for frail seniors. Since 1987, Bienvivir has served the frail seniors of El Paso, Texas through the provision of the Program of All-Inclusive Care for the Elderly ("PACE").

PACE is a unique managed care benefit for frail seniors (referred to as participants) age 55 and older who are certified by the state as needing nursing home level care and who reside in a PACE service area. PACE programs coordinate and provide comprehensive medical and support services so that participants can remain independent and stay in their homes for as long as safely possible.

Bienvivir is currently accepting applications for the following position:

GENERAL DESCRIPTION:
The HH Skilled Nursing LVN, is primarily responsible for providing services to participants in their homes who require skilled nursing care. Nursing care is provided under the PCP's orders and under supervision of a Registered Nurse. The HH Skilled Nursing LVN, as per the HH department needs, will assist with the HH LVN Coordinator duties under the supervision of the HH Nursing Supervisor.

RESPONSIBILITIES:
1. Provides direct nursing care to participant according to the PCP's Plan of Care under the supervision of the registered nurse. 
2. Documents care provided, and the participant's/family's response to nursing interventions. 
3. Reports significant observations to the PCP and/or registered nurse. 
4. Instructs the participant and family members in appropriate aspects of the participant's care, including the use of written instructions when indicated. 
5. Participates in evaluating the participant's progress and provides input into revising the nursing care plan as appropriate under the PCP's direction. 
6. Recommends discontinuation of nursing and/or personal care services to the RN responsible for the participant based on the evaluation of the participant's progress and goal achievement. Participates in plans for participant discharge from home health services.
7. Participates in and contributes to in-service educational programs and meetings.
8. Demonstrates an understanding of Bienvivir Home Health rules and regulation. Applies knowledge appropriately to documentation and recommendations regarding participant services.
9. Completes and submits required paperwork in a timely manner according to Bienvivir' s HH policies. Posts MSUR home visits within 24 hours of service delivered.
10. Complies with Home Health department polices and demonstrates professional participation on the home care team. 
11. Reports daily to the RN supervisor giving a summary of the day's work.

12. Assist supervisors with assuring participants are assigned the appropriate personnel to meet the level of care needs assessed for each participant. 
13. Meets daily with supervisor to review status of cases and schedules. 
14. Responsible for assigning participants' care to nurses under the direction of the HH Nursing Supervisor.
15. Receives Skilled Nursing orders from clinics and verifies in participant's EMR if the visit would be SOC, Re-assessment or Routine visit. 
16. Assures proper documentation submitted in timely manner. 
17. Responsible for the Skilled Nursing and Comfort Care on-call schedule. Responsible to find coverage for shifts and notification of any changes to the MDXchange and Home Health Department. 
18. Responsible to assign all schedule SN visits and verify completion.
19. Have weekend schedule ready no later than Wednesday with assistance and review by HH Nursing Supervisor.
20. Other duties and responsibilities as delegated by the HH Nursing Supervisor and/or HH Administrator.

QUALIFICATIONS / REQUIREMENTS:
1. Graduation from a State approved school of Nursing. 
2. Currently licensed (in good standing) through the State Board of Texas as a Licensed Vocational Nurse. 
3. Meets hiring criteria as defined in Bienvivir Senior Health Services' standards. 
4. TB test or chest X-ray
5. Current CPR card. 
6. Current driver's license and automobile insurance
7. Bilingual in English and Spanish.
