Hospice Admissions Nurse - Full Time - 5K Sign On Bonus

The Admission Nurse is responsible for the care of patients assigned by the Director of Hospice Admissions. The RN assigned will do the admission assessment for that patient, identifying patient needs and status. The RN, under the physician's nursing order, will devise a Plan of Care to address all patient needs to provide optimal functioning.

Job duties include presenting hospice services and philosophy of care to patients and families, performing in-person assessments to determine eligibility for hospice, completing admissions process, initiating plan of care, communicating with the receiving interdisciplinary team, providing direct nursing care in a Hospice setting, performing/completing admission/start of care (SOC), recertification, evaluations, discharges and accompanying documentation, performing a detailed assessment resulting in the development of a patient specific Plan of Care, communicating with other team members regarding the Plan of Care, effectively communicating with referring physician, documenting information regarding interpretation of Hospice care policy, preparing for patient discharge planning, making recommendations for involving other services, advising and consulting with the patient's family in the patient's therapy program, attending staff meetings, documenting all communication with other personnel, using equipment and supplies effectively and efficiently, maintaining confidentiality of patient information, implementing infection control and safety measures, consistently addressing patients concerns, promoting education for patients, family members and caregivers, demonstrating current knowledge regarding professional career, attending mandatory in-services, maintaining compliance with mandatory data-based training, participating in orientation of Hospice/Home Health care staff members, and immediately reporting abuse/suspected abuse.

Job requirements include being a graduate of an accredited school of nursing, licensed to work as a Registered Nurse in the Commonwealth of Virginia, having at least one year acute care experience, Hospice preferred, holding a current CPR card, ability to communicate effectively in English, both verbally and written, knowledge of the duties and functions of Home Health and Hospice care and its relationship to the healthcare community, willingness to learn and perform documentation and SOC, ability to make independent decisions and work independently, possessing a driver's license, proof of automobile insurance and readily available, reliable transportation, ability to deal tactfully and compassionately with patients, family members and caregivers, and willingness to participate in a teamwork approach, working with all disciplines involved in the patient's care/case.
