Position Summary

The Clinical Assessment Registered Nurse (RN) performs comprehensive in-home clinical assessments, admissions, Start of Care (SOC), follow-up visits, reassessments, recertifications, supervisory visits, and discharges.

The Assessment RN uses strong clinical judgment and assessment skills to evaluate patient needs, establish appropriate plans of care, communicate with physicians and the interdisciplinary team, and ensure accurate and timely documentation.

What You Will Do
  • Complete comprehensive in-home nursing assessments and admissions.
  • Perform Start of Care (SOC), follow-up, reassessment, recertification, supervisory, and discharge visits.
  • Assess patient condition, functional status, medications, risks, and ongoing care needs.
  • Develop and update individualized plans of care.
  • Identify changes in patient condition and communicate appropriately with physicians and the clinical team.
  • Educate patients and caregivers regarding diagnoses, medications, treatments, disease management, and safety.
  • Coordinate care with physicians, therapists, pharmacies, caregivers, and agency staff.
  • Ensure physician orders and plans of care accurately reflect patient needs.
  • Complete accurate, thorough, and timely clinical documentation in the agency's EMR.
  • Maintain compliance with VDH, DMAS, Medicare/Medicaid requirements as applicable, HIPAA, and agency policies.
  • Participate in case conferences, clinical meetings, orientation, and continuing education as needed.

  • Current, unrestricted Virginia Registered Nurse (RN) license.
  • Current CPR/BLS certification.
  • Active professional malpractice insurance.
  • Minimum 1 year of professional nursing experience; home health experience preferred.
  • Valid driver's license.
  • Reliable personal transportation.
  • Current auto insurance.
  • Ability to travel throughout the assigned service area.
  • Ability to work independently in patients' homes.

  • Excellent verbal and written communication skills.
