What is a SNF billing?

A SNF (Skilled Nursing Facility) Billing job involves managing the billing and reimbursement processes for services provided in a skilled nursing facility. Responsibilities include verifying insurance coverage, submitting claims to Medicare, Medicaid, and private insurers, and ensuring compliance with healthcare regulations. SNF billers also handle claim denials, appeals, and payment postings to maintain accurate financial records. Strong knowledge of medical coding, insurance policies, and regulatory requirements is essential for this role.

What are the key skills and qualifications needed to thrive in the SNF billing position, and why are they important?

To thrive in SNF Billing, you need strong knowledge of Medicare/Medicaid billing guidelines, medical terminology, and claims processing, usually supported by a background in healthcare administration or billing certification. Familiarity with electronic billing systems, insurance portals, and practice management software such as PointClickCare or MatrixCare is typically required. Attention to detail, problem-solving abilities, and effective communication with both clinical staff and payers are standout soft skills. These are essential to ensure accurate and timely reimbursement, compliance with regulatory requirements, and efficient coordination between facilities and insurance providers.

What are some common challenges faced in SNF billing positions, and how can they be managed?

SNF Billing professionals often encounter challenges such as navigating complex and frequently changing insurance regulations, resolving claim denials, and ensuring accuracy with high volumes of resident accounts. Managing these challenges effectively requires staying updated on payer guidelines, maintaining meticulous documentation, and working closely with both clinical staff and external insurance representatives. Many organizations provide ongoing training and regular team meetings to address regulatory updates and streamline workflow. Being proactive and detail-oriented helps minimize billing errors and speeds up claim resolutions, ultimately supporting the financial health of the skilled nursing facility.

Job description

Behavioral Health Care Manager (LMSW/LCSW) – SNF | CoCM Program

Overview:

We are an established SNF physician group seeking a Behavioral Health Care Manager (BHCM) to support our Collaborative Care Model (CoCM) program across Skilled Nursing Facilities (SNFs) in Georgia. This role focuses on managing residents with depression, anxiety, and other behavioral health conditions through a structured, team-based model in partnership with primary care providers and a consulting psychiatrist.

This is a high-impact, population health role centered on resident engagement, care coordination, and outcomes tracking.

Responsibilities:

  • Conduct resident outreach and follow-up (primarily telephonic/virtual; coordinate with on-site SNF staff)

  • Administer and track PHQ-9, GAD-7, and other screening tools

  • Maintain a behavioral health registry and monitor clinical outcomes

  • Provide brief, evidence-based interventions (behavioral activation, supportive counseling, motivational interviewing)

  • Collaborate closely with SNF nursing staff, PCPs, and care teams

  • Present cases during weekly psychiatric case reviews and implement recommendations

  • Track and document time and services in compliance with CoCM billing requirements

  • Ensure accurate, timely, and compliant EMR documentation

Qualifications:

  • Active Georgia license as:

    • LMSW (under appropriate supervision) or

    • LCSW (independent practice eligible)

  • Experience in behavioral health, SNF/long-term care, or care management preferred

  • Strong communication and interdisciplinary collaboration skills

  • Ability to manage a high-volume caseload

  • Comfortable working across multiple facilities and care teams

  • Proficiency with EMRs and documentation workflows

What Success Looks Like

  • High resident engagement and follow-up compliance

  • Improvement in PHQ-9/GAD-7 scores and behavioral health outcomes

  • Strong collaboration with facility staff and providers

  • Accurate documentation supporting CoCM billing and quality standards

Work Environment:

  • Hybrid role supporting multiple SNFs across Georgia

  • Team-based model including:

    • Primary Care Providers (MD/DO/NP/PA)

    • Consulting Psychiatrist

    • SNF nursing and administrative staff

Compensation & Growth

  • Competitive salary based on licensure (LMSW vs LCSW) and experience

  • Great clinical resources, training and structured supervision available

  • Opportunity to grow within a scalable, team-based behavioral health program

Why Join Us:

  • Help bring behavioral health access to underserved SNF populations

  • Work within an innovative, evidence-based care model

  • Be part of a rapidly growing program with strong clinical support
