Location: New Holstein, WI
Facilities: Willowdale Nursing and Rehab Center & Homestead Health Services
Setting: Skilled Nursing Facility (SNF)

Continuum Therapy Partners is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team serving Willowdale Nursing and Rehab Center and Homestead Health Services in New Holstein, Wisconsin.

This part-time Speech-Language Pathologist opportunity offers the chance to provide exceptional speech therapy services in supportive, collaborative skilled nursing facilities where your clinical expertise makes a meaningful impact every day.

Candidates interested in additional hours may be considered for a full-time opportunity with travel to Plymouth, Wisconsin.

As a Speech-Language Pathologist (SLP), you will evaluate and treat adult and geriatric patients with communication, cognitive, speech, language, and swallowing needs while collaborating with an interdisciplinary rehabilitation team focused on quality patient outcomes.

Responsibilities
  • Evaluate patients and develop individualized speech therapy treatment plans.
  • Provide skilled speech-language pathology services in a skilled nursing facility setting.
  • Treat patients with communication, cognitive, speech, language, and swallowing disorders.
  • Complete swallowing evaluations and provide appropriate interventions as needed.
  • Develop treatment strategies to improve patient safety, communication, and quality of life.
  • Monitor patient progress and adjust treatment plans based on patient needs.
  • Collaborate with physicians, nursing staff, caregivers, and interdisciplinary rehabilitation team members.
  • Complete accurate and timely documentation according to company, state, and federal guidelines.
  • Educate patients, families, and caregivers on strategies to support communication and swallowing safety.
Qualifications
  • Graduate of an accredited Speech-Language Pathology program.
  • Current Wisconsin Speech-Language Pathologist (SLP) license required.
  • Current ASHA certification required.
  • Skilled nursing or rehabilitation experience preferred but not required.
  • New graduates are encouraged to apply.
  • Strong communication, organization, and teamwork skills.
