Hourly Pay Range (Minimum - Midpoint - Maximum):
$42.790 - $55.210 - $61.840
The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant's years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.
What You Will Do
The Medical Social Worker provides patient/family psychosocial consultation, inventions and resources which may include, but not limited to, Social Drivers of Health (SDOH), family and community support systems, substance use, and protective factors. The Social Worker works closely with the RN Case Manager/Transition Planner and interdisciplinary team to ensure safe and timely discharge.
Required Qualifications
  • Master's Degree in Social Work.

Preferred Qualifications
  • Experience in discharge planning or in a hospital setting.
  • Medical social work experience including patient/family counseling.
  • California BBS Licensed Clinical Social Worker (LCSW) - CA Board of Behavioral Sciences -PREFERRED

Essential Functions
  • Collaboration and Communication
    Maintains required social work credentials in good standing.
    Complies with the policies and Everyday Action Behavior Standards of Sharp Healthcare and the NASW Code of Ethics.
    Complies with all operational policies.
    Participates in facility in-service activities that promote greater understanding of case management and social work processes within Sharp Healthcare.
    Demonstrates ongoing willingness to increase knowledge base of social work principles and case management/social work practices specific to Sharp Healthcare.
    Appropriately receives constructive guidance from immediate supervisor.
    Represents the facility to other healthcare professionals, current and potential referral sources and the general public in accordance with the mission and philosophy of Sharp Healthcare.
    Preserves the basic human right of expression, decision, action and personal dignity for all patients/families.
    Respects the cultural diversity of all patients/families and employees of Sharp Grossmont Hospital.
  • Department Improvement
    Provides input into the development and improvement of departmental processes.
    Attends and participates in monthly departmental meetings including case review presentation.
    Demonstrates knowledge of and adherence to departmental processes and policies.
  • Assessment and Documentation
    Completes psychosocial assessments, identifying patient needs related to suicidality, social determinants of health, protective factors, substance use, mental health, family dynamics and barriers and goals related to safe discharge.
    Documents assessments, interventions, discharge plans and barriers in EMR per Sharp policies.
    Collaborates with RN Transition Planner in formulating discharge plan and addressing barriers and goals.
    Escalates cases with barriers discharge, or complex needs, to department leadership.
  • Social Work Consultation
    Provides appropriate support to patient/family within the scope of social work practice.
    Addresses social work consults received, including, but not limited to, those related to social determinants of health, protective factors, family dynamics, end of life, medical decision making, conserved patients, substance use and mental illness.
    Provides updates to interdisciplinary team related to social needs that may affect the inpatient stay and/or discharge plan.
    Maintains ongoing communication throughout admission with patient/family and interdisciplinary team.
    Attends and documents unit rounds as required.
    Attends, and schedules when needed, family conferences related to high risk or difficult social situations.
    Per Sharp Healthcare Policy, reports identified mandated protective issues to appropriate agencies including Adult Protective Services, Child Welfare Services, Community Care Licensing, Ombudsman and Department of Public Health.
  • Referral Management
    Manages referrals and consults received, prioritizing those related to patient safety.
    Provides community resources and referrals to address social needs identified in assessment.
    Provides hand-offs to team and colleagues to ensure continuity of care.
    Repeatedly evaluates, at appropriate intervals, the effectiveness of the discharge plan in reaching the desired outcomes and goals while modifying any or all components of as necessary.
