Case Manager
$20.99 - $23.49/ hour Work-Life Balance Work to make an impact!
The Shared Living Case Manager coordinates and monitors individualized placements, ensuring the health, safety, and personal goals of individuals served. Responsibilities include case management, skill training, supportive counseling, crisis intervention, home visits, service planning, caregiver support, and accurate documentation.
The Case Manager collaborates with individuals, families, caregivers, healthcare providers, and community partners while ensuring compliance with state regulations, funding requirements, and agency standards.
Schedule: Monday-Friday, 8:00 AM-4:00 PM
Mileage reimbursement and regional differentials offered.

Required Qualifications:
  • Minimum of 2 years' experience in case management, direct support, or a related human services role.
  • Knowledge of person-centered planning, developmental disabilities, and community resources.
  • Strong organizational, documentation, and communication skills.
  • Ability to work independently while maintaining collaborative relationships with teams and caregivers, and community partners.
  • CPR/First Aid Certified Trainer (or ability to obtain certification within designated timeframe).
  • Valid driver's license, reliable transportation, and willingness to travel extensively.
