Description

The Community-Based Care Coordinator (CCS Service Facilitator) plays a vital role in providing trauma-informed, empowerment-based care coordination to individuals with high-level mental health and/or substance use needs. This role focuses on delivering psycho-social rehabilitative services in diverse settings, including homes, schools, and community spaces, to help clients achieve recovery and self-determination.

Under the guidance of the Mental Health Services Supervisor, the Service Facilitator will conduct mental health assessments, develop personalized recovery treatment plans, and facilitate psycho-social rehabilitation to reduce the negative impacts of mental health and substance use disorders. The ideal candidate is passionate about social justice, cultural humility, and trauma-informed care, with a strong commitment to serving marginalized communities, including survivors of sexual trauma, clients of color, LGBTQIA+ individuals, and other underserved populations.

This role requires empathy, adaptability, and a deep understanding of privilege, oppression, and systemic barriers affecting client needs. The Service Facilitator will collaborate closely with a multidisciplinary team, maintain accurate records, and contribute to continuous improvement in service delivery.
 

Key Responsibilities Client-Centered Care & Support
  • Provide psycho-social rehabilitative services that promote self-determination, empowerment, and recovery for clients of all ages.
  • Conduct mental health assessments, including suicidal and homicide risk assessments, and develop personalized recovery treatment plans in collaboration with clients.
  • Facilitate individual and group psychoeducationskill development, and trauma-informed support tailored to the unique needs of marginalized survivors, including considerations for race, ethnicity, gender, gender identity, religion, age, ability, and other intersecting factors.
  • Ensure culturally sensitive and trauma-informed service delivery, with a focus on marginalized survivors of sexual trauma, clients of color, LGBTQIA+ individuals, and other underserved communities.
  • Maintain accurate and up-to-date client records, including recovery plans, case notes, and master client lists in compliance with agency policies and RCC Helpline training requirements.
Collaboration & Professional Development
  • Participate in individual and group supervisionweekly case reviews, and team meetings to ensure alignment with best practices and continuous improvement.
  • Provide regular reports to the Mental Health Services Supervisor on service gaps, survivor needs, areas for improvement, and professional development opportunities.
  • Stay current with applicable statutes, administrative codes, and agency policies to ensure compliance and high-quality service delivery.
  • Attend regular staff meetings, in-services, and professional development opportunities to enhance skills in trauma-informed care, cultural humility, and empowerment-based models.
Administrative & Operational Support
  • Keep the calendar up to date in Outlook and ensure timely documentation of services provided.
  • Enter CCS case notes into the county module weekly and maintain organized records for audits and client follow-ups.
  • Assist in office operations and contribute to a collaborative, supportive work environment.
  • Perform other duties as assigned to support the mission and goals of the organization.
Qualifications & Skills Required Qualifications
  • Education: Bachelor's or master's degree in Social Work, Counseling, Psychology, or a related field.
  • Experience: Demonstrated experience providing trauma-informed, empowerment-based services to individuals with mental health and/or substance use disorders, with a strong preference for experience serving survivors of sexual trauma, clients of color, LGBTQIA+ individuals, and other marginalized communities.
  • Licensing & Training: Commitment to maintaining licensing and training records as required by state regulations.
  • Cultural Competency: Clear understanding of privilege, oppression, and systemic barriers affecting client needs, with a commitment to cultural humility and trauma-informed care.
  • Communication: Excellent verbal and written communication skills, with the ability to document services clearly and professionally.
  • Flexibility: Ability to work flexible hours, including evenings or weekends, as needed to meet client needs.
  • Background Check: Satisfactory criminal background check.
  • Confidentiality: Commitment to maintaining client confidentiality and adhering to ethical standards in all interactions.
Preferred Qualifications
  • Bilingual (Spanish/English) proficiency.
  • Knowledge of Dane County community resources and local support systems.
  • Personal or professional experience in communities of color or other marginalized communities.
Work Environment & Physical Requirements
  • Primary Work Setting: Office, classroom, or community-based environments.
  • Physical Demands: Ability to lift and move up to 20 pounds, perform daily movement (e.g., walking, standing), and use computers, office equipment, and telephones.
  • Work Conditions: Frequent interruptions, low to moderate noise levels, and dynamic environments requiring adaptability.
