The Vineyards Healthcare Center is a skilled nursing facility located at 76 Fenton Street, Livermore, California. It offers 24-hour Medicare-certified care in an 83-bed facility near downtown Livermore. It has received high-performing ratings in both short-term rehabilitation and long-term care from the U.S. News & World Report. Position Overview:

We are seeking a highly motivated and experienced Director of Social Services to lead and oversee our social service programs. The successful candidate will be responsible for planning, developing, organizing, implementing, evaluating, and directing social service initiatives in accordance with current federal, state, and local standards, guidelines, and facility policies and procedures. This role ensures that the medically related emotional and social needs of each resident are met and maintained on an individual basis.

RESPONSIBILITIES:

  • Develop, implement, and oversee social services programs in the skilled nursing facility.
  • Conduct resident assessments to identify their social, emotional, and psychological needs.
  • Collaborate with the healthcare team to create individualized care plans for residents.
  • Facilitate family meetings to communicate resident progress and address concerns.
  • Act as an advocate for residents’ rights and ensure compliance with relevant regulations.
  • Train and supervise social service staff, providing guidance and support as needed.
  • Engage with community resources and organizations to enhance services for residents.

QUALIFICATIONS:

  • Previous experience as a Social Service Director in a Skilled Nursing Facility for a year is a must
  • A minimum of 3 years of experience in a social service role, preferably in a healthcare or skilled nursing setting is a plus.
  • A state driver's license is a plus
  • Strong understanding of social and healthcare policies affecting patient care.
  • Excellent communication and interpersonal skills for working with residents, families, and staff.
  • Ability to work independently as well as collaboratively in a team-oriented environment.
  • Proficient in Microsoft Office Suite and electronic medical records systems.
