About the Role

The Care Partner supports patients in facility-based settings by providing patient engagement, behavioral health support, care coordination assistance, and communication support between patients, providers, facilities, and the EverCare team. This is a mobile role serving assigned facilities in the Eugene/Springfield area.

What You'll Do

Build positive, supportive relationships with patients in long-term-care and assisted-living settings.

Support patients with engagement, emotional regulation, social connection, and follow-through.

Observe and report patient needs, changes, or concerns to the appropriate EverCare team member.

Communicate professionally with facility staff and internal team members.

Support continuity of care by helping ensure patients are seen, engaged, and followed up on.

Complete timely and accurate documentation.

Follow assigned workflows, care plans, and escalation procedures.

Protect patient privacy and follow HIPAA expectations.

Participate in required training, meetings, and team communication.

What We're Looking For

Experience in caregiving, behavioral health, senior living, memory care, DSP, case aide, care coordination, or a related role.

Strong communication, reliability, and follow-through.

Comfort working independently in the field and traveling locally between facilities.

Ability to build trust with older adults and vulnerable patients.

Calm, patient, and respectful approach to behavioral health needs.

Reliable transportation and ability to meet facility access requirements.

Comfort using mobile apps, EHR systems, Slack/email, and online forms.

Ability to pass required background checks and facility requirements.

Preferred Qualifications

QMHA-R or eligibility for QMHA-R.

Bachelor's degree in psychology, social work, counseling, human services, or related field.

Dementia, Alzheimer's, memory care, or behavioral health experience.

Experience working in assisted living, skilled nursing, adult foster care, or community behavioral health.

Bilingual skills.

Schedule

Flexible full-time or part-time options may be available depending on facility/patient needs and candidate availability. Standard work is generally during daytime business hours, with some flexibility based on patient and facility schedules.
