Home Health Quality Assurance (QA) Nurse

Center of Illinois Home Healthcare, Inc. is seeking an experienced Home Health Quality Assurance (QA) Nurse to join our growing team.

We are a Medicare-certified, ACHC-accredited home health agency focused on delivering high-quality patient care while maintaining strong compliance and clinical standards throughout our organization.

This position is ideal for a clinician who enjoys reviewing documentation, improving accuracy, educating staff, and helping maintain operational excellence within a fast-paced home health environment.

What You’ll Do:
• Review OASIS assessments and clinical documentation for accuracy and compliance
• Ensure records meet CMS, Medicare, ACHC, and agency standards
• Review Plans of Care and clinical documentation for completeness
• Communicate corrections and documentation feedback to clinicians
• Collaborate with QA, coding, intake, and clinical leadership teams
• Assist with quality improvement and audit readiness initiatives
• Monitor documentation timeliness and compliance metrics
• Support clinician education and documentation improvement efforts
• Help maintain efficient clinical workflow processes

What We’re Looking For:
• Home Health experience required
• Strong OASIS and Medicare knowledge required
• QA review experience preferred
• Active Illinois RN / LPN license preferred
• Experience with Axxess EMR preferred
• Strong communication and organizational skills
• Ability to multitask and meet deadlines in a fast-paced environment
