Full-time Remote Position 

MUST LIVE IN DRIVING DISTANCE TO INDIANAPOLIS, INDIANA 

Monday - Friday

What does a Transitional Care Coordinator do each day at ASC?

  • Provides customers with facility-related information via telephone conversations.
  • Process new business leads promptly in our referral management system.
  • Participate in ongoing communication with our facilities regarding leads.
  • Held accountable for results and should display a sense of urgency when processing leads.
  • Supply accurate information to the best of their knowledge per the data entry expectation list.
  • Knowledgeable of different payer types across different levels of senior care.
  • Perform benefit verifications accurately using different insurance portals. 

    Requirements

    • Previous health care admissions/marketing/sales experience preferred.
    • Experience with verifying insurance benefits preferred
    • Demonstrates C.A.R.E. values to our residents, family members, customers and staff
    • Must be willing to work flexible hours, including evenings and weekends
