• Conduct comprehensive psychosocial assessments to accurately diagnose the mental health conditions of patients within the hospice setting, utilizing your clinical expertise and thorough evaluation techniques.
• Develop individualized treatment plans that address the specific emotional, psychological, and social needs of patients and their families, creating a pathway for coping during this challenging time.
• Implement evidence-based behavioral therapies, including solution-focused therapy, motivational interviewing, and cognitive-behavioral techniques, ensuring that interventions are outcome-focused and tailored for patients' unique circumstances.
• Work collaboratively with a multidisciplinary team, including primary care physicians, nurses, and psychologists, to ensure a holistic approach to patient care, fostering open communication about patient progress and treatment effectiveness.
• Regularly monitor patient responses to therapeutic interventions through virtual sessions, adjusting treatment strategies as necessary, and celebrating progress with patients and families to enhance motivation.
• Assist in managing psychotropic medications prescribed by medical providers, focusing on adherence, side effect monitoring, and evaluating the overall effectiveness of treatment plans.

The primary focus of this position is to support patients receiving hospice care, many of whom are dealing with complex emotional challenges associated with terminal illness. You'll engage with patients and their families to provide comfort, guidance, and support during one of life's most difficult transitions, ensuring that their mental health needs are addressed with dignity and respect.

• A Master’s degree in Social Work from an accredited program is required.
• Must possess a current, valid, and unrestricted license to practice independently as a Licensed Clinical Social Worker (LCSW) in New York.
• At least 1 year of documented experience as an independently licensed therapist, ideally in healthcare settings, is essential.

• Experience working within hospice or palliative care environments is highly desirable.
• Familiarity with telehealth platforms and virtual therapy techniques will be advantageous.
• Demonstrated ability to work collaboratively in a multidisciplinary team setting.

The salary range for this position is competitive, offering between USD 80,000 and 100,000 annually, complemented by a performance-based incentive bonus. In addition to a robust salary, employees will enjoy a comprehensive benefits package including:
