Join our dedicated team at Second Chance Health Center as a Senior Licensed Clinical Social Worker (LCSW). In this full-time, remote role, you will play a vital role in enhancing patient care through comprehensive assessment and support. You will utilize telehealth technology to provide mental health services that focus on improving emotional and social well-being for individuals facing substance use disorders and trauma, particularly in a hospice care setting.

Work Environment

This remote position allows LCSWs to engage with clients through advanced telecommunication technologies. You will collaborate with a multidisciplinary team including medical professionals, case managers, and support staff. Key Responsibilities
  • Conduct thorough assessments of patients and their families to evaluate social, emotional, and economic factors that impact healthcare access and quality.

  • Utilize telehealth platforms effectively to deliver counseling sessions, ensuring support and continuity of care for clients remotely.

  • Respond proactively to crisis situations, including mental health emergencies, collaborating with local authorities and mental health services when necessary.

Patient Population / Client Focus

In this role, you will primarily work with patients receiving hospice care, focusing on individuals with life-limiting conditions and their families. You will provide essential emotional support to enhance their quality of life, helping clients and loved ones cope with end-of-life challenges. Special attention will be given to veterans, ensuring that you leverage your skills to meet the unique needs of this population.

Qualifications
  • A Master's degree in Social Work (MSW) from an accredited institution.
  • A valid California LCSW license is mandatory.
  • A minimum of 2 years of experience in providing mental health services, particularly within a healthcare or hospice care setting, is required.
  • Experience in providing services to veterans and understanding the unique challenges they face is highly preferred.
  • Familiarity with Department of Mental Health billing practices and associated paperwork is essential.
  • A strong foundation in parent-child intervention strategies and knowledge of child development principles will be beneficial.
  • Exceptional verbal and written communication skills, with the ability to effectively counsel and empower patients and their families.

This position offers a competitive salary ranging from USD 120,000 to 130,000 annually.
