Purpose

The Social Worker assists with the implementation and evaluation of Social Services programs and departmental operations, ensuring compliance with all applicable federal, state and local regulations and standards.

Strategy & Focus

Administrative 

  • Implement the Social Services programs of the facility under the direction of the Administrator
  • Participate in the facility assessment and assess individual Social Services needs and available resources
  • Assist in obtaining resources from community social, health, and welfare agencies to meet resident needs
  • Coordinate Social Services activities with other members of the interdisciplinary team (IDT)
  • Assist residents, representatives and families with discharge planning (including coordination with community agencies), and referrals
  • Ensure each resident receives necessary behavioral health care and services to attain and maintain the highest practical physical, mental and psychosocial well-being in accordance with the comprehensive assessment and plan of care
  • Participate in resident and group council meetings
  • Provide residents with information regarding resident rights, advance directives, living wills and related matters
  • Maintain current knowledge of federal and state regulations and professional standards and recommend policy and procedure changes
  • Coordinate resources and assist residents with limited English proficiency (LEP), including verbal and written translation services when applicable
  • Maintain a reference library of written materials, laws and standards of practice necessary to ensure regulatory compliance and quality Social Services
  • Meet with administration, medical, nursing and other department staff to plan and coordinate Social Services programs and activities
  • Participate in facility surveys conducted by authorized government agencies

Care Plan & Assessment 

  • Participate in the development of resident-centered care plans
  • Involve residents and families in establishing individualized goals and objectives
  • Communicate residents’ social, psychological and emotional needs to other members of the IDT
  • Complete and sign required Social Services sections of the Minimum Data Set (MDS), Brief Interview for Mental Status (BIMS), or other cognitive assessments as applicable
  • Collaborate with the IDT to prioritize non-pharmacological interventions for resident behaviors in accordance with facility policy and best practices

Admission, Transfer & Discharge 

  • Identify resident discharge needs and ensure development of an appropriate discharge plan
  • Coordinate resident transfers and discharges
  • Manage the transfer or discharge process when a resident has filed an appeal
  • Assist residents and their representatives in selecting postdischarge care providers and arranging necessary services
  • Refer residents and families to appropriate community resources when services are not provided by the facility

Personnel

  • Assist in standardizing methods and procedures for completing Social Services tasks

Staff Development 

  • Attend and participate in continuing education programs to maintain licensure and professional competence

Equipment & Supply 

  • Follow established procedures for proper use of labels and Safety Data Sheets (SDS)
  • Maintain a clean, organized and sanitary work environment
  • Follow infection prevention and control procedures
  • Participate in Life Safety Code and emergency preparedness drills
  • Report all occupational injuries and illnesses to the appropriate supervisor promptly

Budget & Planning 

  • Conduct Social Services programs and activities within the established departmental budget

Education

  • Bachelor Degree in Social Work

Experience

Required

  • One year of supervised social work experience in a health care setting and working directly with individuals
