Coverage Area: South County
Pay Range: $50.00 - $65.00 per hour

Position Summary:
We are seeking a compassionate and experienced Registered Nurse (RN) to provide weekend home health services across the South County region. This role involves planning, organizing, and coordinating care, with a strong emphasis on community health and patient education.
Responsibilities:
  • Develop and initiate individualized plans of care in collaboration with patients, caregivers, and physicians
  • Deliver preventative and rehabilitative nursing procedures as ordered
  • Educate patients and caregivers on health management and care techniques
  • Identify and implement discharge planning needs
  • Coordinate necessary equipment and services for patient care
  • Supervise LPNs/LVNs and Home Health Aides
Requirements:
  • Active California RN license
  • Minimum 2 years of direct patient care
  • Minimum 1 year of home health experience required
  • Current CPR certification
  • Valid driver's license and insured, reliable vehicle
