National Outreach Manager
Boca Recovery Center
Boca Recovery Center is seeking a National Outreach Manager to support our Outreach and Admissions teams by managing referral relationships, coordinating inbound and outbound patient placements, and ensuring continuity of care throughout the treatment process.
This role plays a key part in strengthening our national referral network while ensuring patients are connected to the appropriate level of care both within Boca Recovery Center and with trusted partner programs.
The ideal candidate will have experience in addiction treatment admissions, outreach, or partnership management, along with a strong understanding of the continuum of care in substance use treatment.
Requirements
Key Responsibilities
Referral Management
  • Oversee and support the management of inbound referrals from external treatment centers.
  • Collaborate closely with the call center and admissions team to ensure timely follow-up and a smooth intake process.
  • Maintain strong communication with referral partners to ensure high-quality coordination and patient care.
Outbound Referral Coordination
  • Coordinate outbound referrals for patients who cannot be admitted due to capacity, insurance limitations, or clinical needs.
  • Work with national referral partners to identify appropriate placements for patients.
  • Serve as a primary point of contact for referral partners and referred clients during transitions of care.
Aftercare & Stepdown Coordination
  • Track patient stepdowns following detox or residential treatment.
  • Collaborate with clinical and facility teams to ensure appropriate transitions to lower levels of care such as PHP or IOP.
  • Maintain accurate documentation of stepdowns and referral outcomes within Salesforce.
Reporting & Data Management
  • Generate reports from Salesforce related to referral sources, trends, and partnership activity.
  • Monitor referral conversion rates and identify opportunities for improvement.
  • Maintain accurate and up-to-date referral and patient data within CRM systems.
Cross-Department Collaboration
  • Work closely with the National Outreach team, Admissions leadership, and call center staff.
  • Identify and resolve issues within the referral and admissions pipeline.
  • Communicate trends, concerns, and partnership opportunities to leadership.

Qualifications
Education
  • High School Diploma or GED required
  • Associate degree preferred
Experience
  • Experience in admissions, outreach, business development, or partnership management within healthcare or addiction treatment.
  • Strong knowledge of the addiction treatment continuum of care, including detox, residential, and stepdown programs.
  • Experience using Salesforce or similar CRM systems.
Skills
