Job Summary
We are seeking a Registered Nurse Supervisor to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest.ย  Identifies and evaluates client needs and appropriateness of home care byย  reviewing the initial intake screening form before assignment of direct care staff . Assists with the orientation of new professional staff to familiarize them with Agency policies and procedures.
To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.ย 
Responsibilitiesย 
  • Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plansย 
  • Administer medications and monitor patients for side effects or reactions ย 
  • Document any changes in patient conditions and report on changesย 
  • Maintain accurate records of patient care and discuss observations with the supervisor ย 
Qualifications
  • Currently licensed Registered Nurse (RN) in state of employmentย 
  • Previous experience as a Registered Nurse is preferred
  • Geriatric or Home Care experience is preferred
  • Excellent interpersonal and client care skills
  • Valid driverโ€™s licenseย 
  • First aid/CPR certification requiredย
