Job Summary
You will be directly working with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role.
Responsibilities 
  • Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
  • Provide attentive personal care and companionship
  • Care for patients by changing bed linens, doing laundry, cleaning the home, and assisting with personal hygiene and grooming
  • Prepare meals and assist with feeding when needed
  • Run errands such as grocery shopping, picking up prescriptions, and attending appointments
  • Maintain records of patient care, condition, progress, or problems to report observations to the supervisor
  • Build positive relationships with patients by conversing, reading aloud, or engaging in meaningful activities to keep them mentally healthy and alert
  • Provide medication reminders
  • Assist with mobility, transfers, and light exercises
  • Monitor patient safety and overall well-being
  • Assist with toileting and incontinence care when necessary
  • Support patients with daily routines and independent living skills
  • Escort clients to appointments or community activities when needed
  • Provide light housekeeping and homemaker services to maintain a safe and comfortable environment
  • Offer emotional support and companionship to clients and families

Qualifications
