Position Overview

We are seeking a compassionate and detail-oriented Registered Nurse (RN) to join our home health team. In this role, you will provide skilled nursing care to patients in their homes, helping them manage acute and chronic conditions while promoting independence and improving overall health outcomes.
Key Responsibilities

  • Perform comprehensive patient assessments and develop individualized care plans
  • Provide skilled nursing services including wound care, medication management, IV therapy, and disease education
  • Monitor patient conditions and report changes to physicians and care team
  • Educate patients and caregivers on treatment plans, medications, and self-care techniques
  • Coordinate care with physicians, therapists, and other healthcare professionals
  • Complete accurate and timely documentation in compliance with regulatory and agency standards
  • Participate in on-call rotation as required
  • Travel to patient homes within assigned service area
Qualifications

  • Current Registered Nurse (RN) license in [State]
