Are you a compassionate and detail-oriented Registered Nurse (RN) looking to make a meaningful impact in home healthcare? Excel Home Care Services is currently seeking a dedicated RN to join our team and provide Medication Management to clients in the comfort of their homes.
Position Overview
As a Medication Management RN, youll play a vital role in helping patients safely manage their medications. Through in-home visits, you'll assess needs, educate clients and caregivers, and collaborate with physicians and pharmacists to support the overall care plan.
Key Responsibilities

  • Conduct comprehensive in-home assessments to manage and review medications
  • Educate clients and caregivers on proper usage, side effects, and compliance
  • Pre-fill and organize medication planners as needed
  • Monitor for adverse reactions and report changes in condition
  • Coordinate with the care team to address any medication-related concerns
  • Accurately document visits, care provided, and patient progress
  • Perform health assessments including vital signs and ongoing care needs
  • Ensure compliance with healthcare standards and agency policies
Qualifications
