Responsibilities for Nurse Concierge:
  • Serve as a welcoming and accessible point of contact for residents and families, providing support, information, and guidance throughout the resident experience.
  • Build positive relationships with residents and families through regular rounding, proactive communication, and attentive follow-up.
  • Assist residents and families in navigating questions or concerns related to care, services, routines, and the overall facility experience.
  • Collaborate closely with nursing, clinical, administrative, and interdisciplinary team members to communicate resident and family needs and support timely resolution of concerns.
  • Use nursing knowledge and sound clinical judgment to identify concerns requiring additional assessment or intervention and promptly communicate them to the appropriate clinical team member.
  • Support new residents and families during admission and transition into the facility, helping them feel informed, comfortable, and welcomed.
  • Follow up on resident and family questions, requests, and concerns to promote clear communication and ensure appropriate resolution.
  • Identify opportunities to enhance resident satisfaction, service delivery, and the overall resident and family experience.
  • Maintain accurate and appropriate communication and documentation in accordance with facility policies and applicable requirements.
  • Protect resident privacy and confidentiality while maintaining professional boundaries and adhering to HIPAA requirements.
  • Maintain a calm, compassionate, and professional presence while managing multiple priorities in a fast-paced healthcare environment.
  • Serve as an ambassador for the facility, consistently demonstrating a commitment to exceptional care, service, and hospitality.
  • Will perform initial assessments.

Qualifications for Nurse Concierge:
  • Current and active nursing license in the state as an LPN or RN required.
  • Nursing experience in a skilled nursing, long-term care, rehabilitation, assisted living, or similar healthcare setting strongly preferred.
  • Strong customer service orientation with a professional presence, positive attitude, and resident-centered approach.
  • Excellent verbal and written communication skills with the ability to communicate clearly and compassionately with residents, families, and team members.
  • Ability to build trust and rapport while effectively addressing questions, concerns, and service needs.
  • Strong clinical judgment with the ability to recognize concerns that require escalation to the appropriate member of the care team.
  • Experience managing multiple priorities and demands simultaneously in a fast-paced environment.
  • Ability to collaborate effectively with nursing, interdisciplinary, administrative, and support teams.
  • Strong initiative, organization, follow-through, and problem-solving skills.
  • Commitment to delivering an exceptional resident and family experience while maintaining resident dignity, privacy, and confidentiality.
