Job Overview:
MTK Healthcare INC. is seeking a compassionate and experienced Registered Nurse (RN) Case Manager for a Hospice assignment with HopeWest Hospice in Montrose, CO. This role is ideal for nurses passionate about providing meaningful, patient-centered care while supporting patients and families through their healthcare journey.
The Hospice RN Case Manager provides compassionate care, clinical expertise, and guidance to patients and caregivers while managing an assigned caseload and leading interdisciplinary care teams.
Position Responsibilities:
โ€ข Provide skilled nursing care to hospice patients in private homes, nursing homes, group homes, and assisted living facilities
โ€ข Perform comprehensive patient assessments and develop individualized care plans
โ€ข Manage assigned patient caseload and coordinate care with interdisciplinary teams
โ€ข Educate patients and caregivers on disease processes, symptom management, and goals of care
โ€ข Provide emotional support and guidance to patients and families
