RN Case Manager Hospice

Location: Merrifield, VA

Shift:Monday - Friday 8:00am - 5:00pm with occasional responsibility for after hours on call shifts and holiday rotations for coverage.

GENERAL DESCRIPTION SUMMARY
The Home Care RN Case Manager provides, coordinates, teaches and supervises the nursing care of Hospice patients and families/caregivers. Working in collaboration with other members of the interdisciplinary team/group, the RN Case Manager plans and manages the nursing component of the hospice caseload. The RN Case Manager makes initial and ongoing nursing assessments, judgments and treatment decisions based on patient and family/caregiver needs and wishes and in accordance with her/his professional skills.

Experience Requirements

Must have one year of staff nurse experience in a comparable setting/unit. Hospice and/or palliative care experience is strongly preferred.

Education Requirements

Must have a degree in Nursing from an accredited nursing program. A BSN degree is preferred.

Required Certificates and/or Licenses

Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice or licensure from compact state. Healthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of Columbia. A CHPN certification is a plus.
