Responsibilities:

The Nurse Practitioner (NP) works in our team-based care environment. We are a value based care provider focused on quality of care for the patients we serve. Our care team consists of doctors, advanced practice professionals, Pharm D, Care Coach Nurse, MA, Behavioral health specialist, Quality-based Coder, Referral Coordinator and more. Our approach allows us to provide an unmatched experience for seniors. Our model positions itself to provide higher quality care and better outcomes for seniors, providing a concierge experience, diverse services, coordinated care supported by analytics and tools, and deep community relationships. This support allows our Nurse Practitioner to see fewer patients and spend more time with them.

Patient Care:

  • Evaluate and treat center patients following standards of care.
  • Follow level of medical care and quality for patients and monitors care using available data and chart reviews.
  • Help coordinate patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment and home health care.
  • Be an active participant and lead with the care team through daily huddles.
  • Help Regional Medical Director (RMD), Physician and Center Administrator in setting a tone of cooperation in the practice.
  • Complete all medical record documentation promptly working with a quality- based coder to improve coding specificity.
  • Follow policy and protocol defined by Clinical Leadership.
  • Meet with RMD about quality of care, review of outcome data, procedure and records issues.
  • Participate in potential growth opportunities for all services within the Center.
  • Participate in the local primary care "on-call" program of CenterWell Senior Primary Care.
  • Assure personal compliance with licensing, certification, and accrediting bodies.

Required Qualifications

Bachelor's degree

Masters in Science of Nursing /PA

Active, unrestricted Nurse Practitioner/PA license in the appropriate state

You will consider patients and be part of Humana/Senior Bridge's Tuberculosis (TB) screening program, which they conduct. If selected for this role, we will require you to undergo TB screening.

Must be passionate about contributing to an organization focused on improving consumer experiences

Must be willing to participate in limited on-call coverage by phone

You will report to the Associate Medical Director

You will practice in an onsite outpatient clinic setting at our Irvington, Indianapolis clinic.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


$123,800 - $170,300 per year


This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
