POSITION SUMMARY

The PAY Case Managerย supports participants aged 12-21 in completing community service hours and achieving personal goals. They coordinate youth program activities and build partnerships with community organizations.

PRIMARY DUTIES AND RESPONSIBILITIES:

Duties and responsibilities of the Pay Case Manager include but are not limited to:ย 

Administrative and Program relatedย ย 

  • Coordinates, communicates and monitors participants' case plan to encourage successful completion of program
  • Oversees scheduling of youth in the program;
  • Documents all contacts and information as per guidelines;
  • Tracks youth's attendance
  • Completes monthly reports;ย 
  • Liaises with community organizations to develop partnerships based on program and participant needs;
  • Implements and adheres to all agency and program policies and procedures;
  • Identifies and communicates concerns and updates regarding community programming;
  • Attends and participates in program meetings.
  • Addresses work centre concerns with appropriate personnel when necessary and in a timely manner;
  • Ensures all adult volunteer mentors, students and new staff receive support, training and directionย  ย 
  • Supervises mentors while working in the community
  • Provides feedback and evaluation to mentors, and students

Participant Related:ย ย 

  • Builds positive relationships and rapport with participants, helping to identify personal strengths to promote a sense of belonging, independence, generosity and mastery
  • Acts as an advocate on behalf of the best interest of youth, ensuring their rights are respected;
  • Makes links and connections with participants in the community to support their skill development utilizing volunteer work and participant goals;
  • Acts as a positive role model for youth;
  • Ensures all participants receive support, encouragement and follow-up.
  • Manages case load, including intake, scheduling and follow up
QUALIFICATIONS:

The successful candidate should demonstrate the followingcompetencies, which incorporate the Seven Sacred Teachings;ย 

  • Post -secondary education in relevant social services or a combination of work and/or life experience
  • Demonstrated knowledge of cultural awareness, anti-oppressive and trauma informed best practicesย 
  • Knowledge and experience with diverse cultures and socio-economic backgroundsย 
  • Knowledge of Strength-based philosophy and solution-oriented approach Understanding of crisis intervention, harm reduction framework and conflict resolution skillsย 
  • Ability to work independently as well as within a team
  • Demonstrated experience in group facilitation;
  • Strong interpersonal skills
  • Ability to be flexible with schedule based on program and participant needs (days, evenings and weekends)ย 
  • Willingness to obtain necessary training as per the organization's training policy including Crisis De-escalation, Applied Suicide Intervention, Colonization Awareness, and Child Developmentย 
  • Emergency First Aid and CPR Level C, or willingness to obtain
  • Satisfactory Criminal Record Check with Vulnerable Sector Search, a clear Child Abuse Registry check with acceptable Prior Contact checks, and a clear Adult Abuse Registry check, all done within the last 3 monthsย 
  • Valid driver's license and acceptable driver's abstract (subject to approval)
  • Access to a personal vehicle for work is required
SALARY RANGE:ย  $20.05 - $23.86 per hour. Depending on the qualification.

HOURS OF WORK:ย  37.5 hours per week - combination of days, evenings, and weekends.ย 12-month term.
