Director of Social Services

At Westerly Rehabilitation and Healthcare Center, we believe that emotional well-being, dignity, and connection are essential components of quality care. Guided by our core values of Passion, Respect, and Excellence, the Director of Social Services provides leadership and oversight of psychosocial services, ensuring residents and families receive compassionate support, advocacy, and guidance throughout their care journey.

Responsibilities for Director of Social Services:

  • Provide leadership and oversight of social services programs to support the emotional, psychosocial, and practical needs of residents and their families.

  • Ensure services are delivered in accordance with all applicable federal, state, and facility regulations, guidelines, and standards of care.

  • Collaborate with interdisciplinary teams to support resident-centered care planning and ensure holistic support for residents’ well-being.

  • Provide compassionate counseling, support, and advocacy for residents and families during admission, care transitions, and end-of-life planning.

  • Serve as a key liaison between residents, families, staff, and external agencies to ensure effective communication and coordinated care.

  • Support and guide staff in addressing resident concerns, behavioral challenges, and psychosocial needs with empathy and professionalism.

  • Assist with discharge planning, resource coordination, and community referrals to support smooth transitions of care.

  • Maintain accurate documentation and records in compliance with regulatory requirements and facility policies.

Qualifications for Director of Social Services:

  • Bachelor’s degree required; Master’s degree strongly preferred.

  • Current LSW/CSW licensure required.

  • Experience as a social worker in a healthcare setting is required.

  • Experience in a nursing home or rehabilitation center strongly preferred.

  • Strong knowledge of regulations, standards, and guidelines governing nursing care facilities.

  • Excellent communication skills with the ability to interact effectively and compassionately with residents, families, staff, and external agencies.
