About the Role
We are seeking a compassionate and reliable Hospice Certified Nursing Assistant (CNA) to support patients served by Fortis Health-affiliated agencies. In this role, you will provide hands-on personal care in the home, focusing on comfort, dignity, and quality of life while assisting patients and families during end-of-life care under the direction of the RN.
Responsibilities
  • Provide personal care including bathing, dressing, oral care, feeding, and elimination
  • Assist with ambulation, positioning, transfers, and ADLs
  • Measure and document vital signs and observations as directed
  • Travel to and care for multiple hospice patients per day in their residences, following individualized plans of care
  • Observe and report changes in patient condition or home safety concerns
  • Follow RN care plans and agency policies
  • Maintain patient confidentiality and HIPAA compliance
  • Complete required trainings, meetings, and in-services
  • Support patient comfort through socialization and emotional presence
