This remote position is set within the supportive framework of our hospice center, allowing LCSWs to work closely with healthcare professionals while offering therapeutic interventions from a conducive home environment or alternative remote workspace. You will have the flexibility to manage your schedule, which may include day, evening, and weekend shifts, depending on departmental needs and patient demands. Additionally, on-call responsibilities may arise, requiring clinicians to respond promptly to emergent psychosocial needs.

• Conduct thorough psychosocial assessments to identify patient and family emotional, social, and financial needs.
• Provide individual and group counseling sessions that focus on coping strategies, grief resolution, and adjustment issues related to medical conditions.
• Collaborate with physicians, nurses, and other care providers to create and implement comprehensive treatment plans that address the psychosocial aspects of care.
• Coordinate with community organizations and resources to ensure continuity of care post-discharge, facilitating smooth transitions for patients.
• Deliver crisis intervention services to patients facing acute emotional distress, instilling hope and providing appropriate coping mechanisms.
• Develop and oversee discharge plans and facilitate connections to community services that enhance post-hospitalization support.
• Maintain clear and accurate clinical documentation in adherence to hospital policies, regulatory requirements, and best practice standards.
• Participate actively in interdisciplinary team meetings and hospital committees, contributing expertise in social work-related issues.

Your work will predominantly focus on patients dealing with anxiety, depression, trauma, and related mental health disorders. You will provide support to patients facing significant health challenges, including those in end-of-life care situations, ensuring they and their families navigate their emotional and psychosocial landscapes effectively. Your expertise will be central to uplifting the patient experience and improving health outcomes.

• A Master's degree in Social Work from an accredited program is required.
• Possession of an active state license as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of healthcare practices, mental health disorders, and crisis intervention strategies.
• Excellent verbal and written communication skills, with a focus on building positive working relationships with both patients and interdisciplinary team members.

• Two or more years of experience working in a healthcare or hospital setting, with a preference for those with hospice or trauma experience.
• Proficiency in electronic health record (EHR) systems to document patient interactions and maintain accurate treatment plans.

The annual salary for this position ranges from $70,000 to $100,000, commensurate with experience and qualifications, alongside a competitive sign-on bonus.
