Key Responsibilities

• Conduct thorough assessments of patients’ emotional and psychosocial needs, tailoring person-centered treatment plans that prioritize their unique circumstances.
• Implement individualized therapeutic interventions, utilizing evidence-based practices to promote mental well-being and coping strategies among patients and their families.
• Maintain accurate and secure electronic records in accordance with HIPAA regulations, documenting assessments, treatment plans, and progress notes.
• Collaborate closely with multidisciplinary teams consisting of physicians, nurses, and other specialists to ensure coordinated and comprehensive care delivery.
• Engage in continuous professional development by staying abreast of the latest research findings and treatment methodologies in hospice care and social work.

Patient Population / Client Focus

The primary focus of this role will be on patients in hospice care, many of whom are facing complex emotional challenges related to terminal illnesses such as cancer, heart disease, and neurological disorders. You will be an integral resource for patients and families, providing advocacy, grief counseling, and assistance in navigating healthcare decisions, thereby improving patient outcomes and experiences.

Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• An active LCSW license in the state of Georgia is mandatory for this role.
• Exceptional communication skills coupled with strong problem-solving abilities, essential for effective patient interactions.
• Ability to thrive in a remote, collaborative team environment, fostering strong relationships with colleagues and clients alike.

Preferred Experience

• Previous experience working with hospice or palliative care populations is highly desirable.

Compensation and Benefits
