Position Overview

Work Environment

As part of our hospice care team, you will collaborate with other mental health professionals remotely to ensure seamless support for clients and their families. Key Responsibilities

• Conduct thorough evaluations and psychosocial assessments to identify clients’ needs and challenges.
• Develop and implement individualized care plans in collaboration with multidisciplinary team members.
• Engage in regular monitoring and evaluation of client progress, adjusting care plans as necessary to ensure optimal outcomes.
• Provide direct counseling services to clients and support families in navigating the complexities of senior care.
• Ensure compliance with all federal and state regulations related to social work and hospice care.
• Facilitate support groups and educational sessions for families, providing resources and coping strategies to manage grief and loss.

Patient Population / Client Focus

Your primary focus will be seniors in hospice care, many of whom face significant life transitions and emotional distress. Understanding the intricacies of each client’s unique situation allows you to provide tailored support aimed at relieving anxiety, fostering resilience, and enhancing overall quality of life. You will also coordinate with family members, helping them to understand and manage their feelings of loss and grief.

Qualifications

• A Master’s degree in Social Work (MSW) or a closely related field.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in New Jersey.
• Expertise in assessment, diagnosis, treatment interventions, risk assessment, and crisis management relevant to geriatric populations.
• Strong verbal and written communication skills to convey complex information calmly and clearly to clients and their families.
• Demonstrated ability to build rapport with seniors, families, and team members, fostering a collaborative and trusting environment.

Preferred Experience

• A minimum of 2 years of experience in clinical social work, with a focus on geriatrics or hospice care.
• Previous experience working in a remote setting is advantageous.
• Bilingual candidates (English/Spanish) are preferred to better serve our diverse client population.

Compensation and Benefits
