Key Responsibilities

• Conduct comprehensive evaluations and psychosocial assessments for clients, identifying needs and developing customized care plans.
• Lead and participate in the development and implementation of Individualized Family Service Plans (IFSPs) that focus on enhancing the quality of life for clients.
• Monitor and evaluate client progress, making necessary adjustments to care plans based on outcomes.
• Provide emotional support and counseling to clients and their families, helping them navigate challenges associated with aging and mental health.
• Collaborate with a multidisciplinary team of professionals to coordinate services and ensure a comprehensive approach to client care.
• Adhere to all federal and state regulations regarding social work practices, ensuring compliance at all times.
• Establish rapport with clients and families to foster trust and promote healthy therapeutic relationships.

Patient Population / Client Focus

Your primary focus will be on the senior population, many of whom may be experiencing complex emotional and psychological challenges. You will address their unique needs arising from aging, including issues related to loss, depression, anxiety, and adjustment to life changes. By understanding the distinct challenges faced by seniors and their families, you will help them improve their quality of life through targeted interventions and support systems.

Qualifications

• A Master’s degree in Social Work (MSW) or a related field is required.
• Current LCSW licensure in California is mandatory.
• Minimum of 2 years of direct clinical experience in social work, specifically with the senior population.
• Expert knowledge in psychosocial assessment, diagnosis, crisis intervention, and treatment methodologies.
• Excellent verbal and written communication skills, enabling clear communication with clients and team members.
• Strong organizational and planning abilities to manage caseload and documentation efficiently.
• Competency in addressing age-related issues and the developmental needs of older adults.

Preferred Experience

• Bilingual proficiency in English and Spanish is highly desirable, as this will enhance communication with a diverse client base.
• Experience in telehealth services is preferred, particularly in adapting counseling techniques for virtual environments.
• Familiarity with community resources and support systems available for seniors in the Oakland area.

Compensation and Benefits

This position offers an attractive salary range of $80,000 to $100,000 annually, commensurate with experience and qualifications. In addition to competitive pay, we offer a robust benefits package that includes:
