Key Responsibilities

• Conduct comprehensive assessments of the psychosocial needs of seniors, integrating emotional, mental, and social factors into care planning.
• Develop personalized care plans in collaboration with clients and their families to address specific goals, preferences, and needs.
• Provide counseling and emotional support for seniors and their families, offering guidance through various life transitions and challenges.
• Advocate for client rights and access to community resources, ensuring that they receive the most appropriate support and services.
• Work alongside nurses and healthcare professionals to coordinate care, including attending virtual meetings to discuss client cases.
• Monitor clients' progress through regular virtual check-ins, adjusting care plans as necessary based on feedback and assessments.
• Facilitate communication among family members to resolve conflicts and encourage a supportive environment for the senior client.
• Educate clients and their families about available community resources, support groups, and programs to enhance engagement and overall well-being.
• Maintain detailed documentation of all assessments, care plans, progress notes, and relevant client interactions.
• Stay up to date with current best practices in social work, ensuring compliance with all ethical and regulatory standards.

As a Licensed Master Social Worker, you will primarily engage with a senior population, particularly those experiencing challenges related to detox services. This demographic often faces complex emotional and social issues that require specialized support. Your role will involve understanding their unique circumstances, advocating for their needs, and providing a safe, supportive space for healing and growth.

Qualifications

• Master's degree in Social Work from an accredited institution.
• Current and valid LMSW licensure in the state of New Jersey.
• A minimum of two years of social work experience, ideally within a community center or geriatric-focused environment.
• Deep understanding of social work principles, particularly as they relate to aging and addiction recovery.
• Strong assessment, counseling, and problem-solving capabilities.
• Genuine compassion for seniors, with the ability to build rapport quickly and effectively.

Preferred Experience

Compensation and Benefits
