Key Responsibilities

As a Remote Social Worker specializing in hospice care, you will engage in a variety of responsibilities aimed at supporting patients and their families. Your key tasks will include:

  • Conducting thorough patient assessments via telehealth to evaluate emotional, social, and psychological needs.
  • Developing and implementing tailored treatment plans that address the unique challenges faced by patients in hospice care.

Qualifications
